About Us

Supporting recovery through lived experience, compassion and purpose.

JC Cornerstone Community Care was created from a place of lived understanding. Shaped by personal challenge, growth and a deep commitment to supporting others, our purpose is to help people move forward with confidence, dignity and hope.

We believe recovery is possible. With the right support, people can build independence, strengthen connections, and create a life that feels meaningful, stable and personally fulfilling.

Founder's Story

A story built on lived experience.

JC Cornerstone Community Care was founded through lived experience and a deep belief in recovery, compassion and paying support forward.

Through my own challenges in navigating life’s ups and downs, I came to understand how important the right support can be. Over time, those challenges became a catalyst for growth. I realised that when I reached out, accepted guidance and stayed open to learning, I began to find my purpose.

That purpose is now reflected in JC Cornerstone Community Care. My experiences have shaped the way I support others: with honesty, realistic expectations, respect and a genuine belief that people can move forward.

I know the value of being heard, encouraged and supported by people who do not give up on you. That is the kind of support we aim to provide — practical, compassionate and grounded in real life.

Every person we support is treated with dignity, respect and care. We are here to listen, encourage and help participants take meaningful steps towards stability, independence and connection.

Together, we can.

Our Purpose

Recovery-focused support with a human touch.

About Psychosocial Support

NDIS psychosocial disability support for recovery, confidence and daily life.

JC Cornerstone has a strong focus on recovery-oriented support for people living with psychosocial disability and mental health-related support needs.

Psychosocial disability refers to the impact a mental health condition can have on a person’s daily life, independence, relationships, routines, confidence and community participation.

A person living with a psychosocial disability may experience challenges in setting goals, making plans, managing daily routines, maintaining social connections, attending appointments, participating in work or education, or staying connected to community and cultural activities.

At JC Cornerstone, our focus is not on labels. It is about understanding the person, their goals, their strengths, and the practical support they need to live with greater stability, independence, and control.

Mental health conditions and support needs

Mental health conditions and support needs

JC Cornerstone supports NDIS participants whose day-to-day life may be affected by psychosocial disability, mental health-related support needs or other functional support needs.

These may include:

  • Schizophrenia

Schizophrenia is a complex mental health condition that may affect a person’s thoughts, perceptions, emotions, communication and sense of reality. Some people may experience hallucinations, delusions, disorganised thinking or difficulties with daily functioning.

NDIS supports may help participants build routine, access services, reduce isolation, attend appointments and strengthen daily living skills.

  • Schizoaffective Disorder

Schizoaffective disorder is a mental health condition that can include symptoms of psychosis, such as hallucinations or delusions, along with mood symptoms such as depression or mania.

Support may focus on building stability, maintaining routines, coordinating services, strengthening coping strategies and helping participants stay connected to their community and support network.

  • Bipolar Disorder

Bipolar disorder is a mood disorder that can involve periods of depression and periods of elevated mood, increased energy or mania. These changes can affect routines, relationships, decision-making, motivation and daily responsibilities.

NDIS support may assist with planning, routines, community access, managing appointments, building independence and staying connected during times of change.

  • Severe Depression

Severe depression can affect a person’s mood, energy, motivation, sleep, appetite, concentration and ability to take part in everyday activities. It may make daily routines, social connections, and personal care more difficult.

Support may help participants rebuild routine, attend appointments, engage in meaningful activities, connect with services and take practical steps towards recovery goals.

  • Anxiety Disorders

Anxiety disorders can involve ongoing worry, fear, panic or avoidance that affects daily life. Some people may find it difficult to attend appointments, use public spaces, manage social situations or take part in community activities.

NDIS supports may assist with confidence-building, community participation, routines, planning and practical strategies for engaging in everyday life.

  • Post-Traumatic Stress Disorder, also known as PTSD

PTSD can develop after a person experiences or witnesses trauma. It may affect sleep, emotional regulation, relationships, trust, concentration, routines and a person’s sense of safety.

Support may focus on practical daily living, building safe routines, connecting with appropriate services, reducing isolation and supporting meaningful community participation.

  • Obsessive-Compulsive Disorder, also known as OCD

OCD can involve unwanted intrusive thoughts and repetitive behaviours or routines that feel difficult to control. These experiences may interfere with daily activities, appointments, relationships, self-care and community participation.

NDIS support may help participants build practical routines, strengthen independence, access services and work towards goals in a respectful and steady way.

  • Agoraphobia

Agoraphobia can involve fear or avoidance of places or situations where a person may feel trapped, unsafe or unable to get help. This may make it difficult to leave home, attend appointments, use transport or participate in the community.

Support may assist with gradual community access, appointments, daily routines, planning and building confidence in safe and practical ways.

  • Borderline Personality Disorder

Borderline personality disorder can affect emotions, relationships, self-image, impulse control and a person’s ability to manage distress. Support needs can vary depending on each person’s situation and goals.

NDIS supports may help with routine, emotional regulation strategies, service connection, communication, daily living skills and building confidence in everyday life.

  • Autism Spectrum Disorder

routines and daily living skills. Each autistic person has different strengths, preferences and support needs.

Where NDIS supports are appropriate, JC Cornerstone may assist with building routines, skill development, community participation, communication support, confidence and connection with suitable services.

  • Other psychosocial or functional support needs

Some participants may not identify with a specific diagnosis, or they may have multiple conditions that affect daily life in different ways.

JC Cornerstone focuses on the person’s functional support needs, goals and approved NDIS plan. We work with participants to understand what support is needed in real life and how services can help build confidence, independence and connection.
