Terms & Conditions
Introduction
These Terms & Conditions apply to services provided by JC Cornerstone Community Care Pty Ltd. By engaging with our services or using our website, you agree to these Terms & Conditions.
These terms are designed to support transparency, informed choice, and respectful service delivery in line with the National Disability Insurance Scheme (NDIS).
Service Provision
JC Cornerstone Community Care Pty Ltd provides NDIS-funded supports in accordance with participant plans, service agreements, and applicable NDIS legislation.
All services are subject to availability and eligibility. Engagement in services is not guaranteed until confirmed in writing.
Pricing and Payments
All pricing is aligned with the current NDIS Pricing Arrangements and Price Limits, which are updated annually.
Pricing details will be discussed transparently prior to service commencement. A detailed pricing schedule is available upon request.
Services are billed according to the agreed service agreement and funding management arrangements.
Travel
Where face-to-face supports are provided, travel may be charged in line with NDIS Pricing Arrangements. Travel charges will be discussed and agreed upon prior to service delivery.
Cancellations
For scheduled face-to-face supports, a minimum of 48 hours’ notice is required for cancellations.
Where sufficient notice is not provided, cancellation charges may apply in accordance with NDIS guidelines. Charges may be applied for the full duration of the scheduled support.
Where appropriate, administrative tasks or agreed objectives may still be completed during a cancellation period.
Meet & Greet Sessions
Initial Meet & Greet sessions are provided at no cost. These sessions are intended to assess suitability, discuss needs, and support informed choice prior to engagement.
Participant Rights and Responsibilities
Participants have the right to:
- Be treated with dignity and respect
- Make informed choices about their supports
- Access information about their services
- Raise concerns or complaints without fear of disadvantage
Participants are responsible for:
- Providing accurate and up-to-date information
- Treating staff with respect
- Notifying us of changes to circumstances or plans
Privacy
We are committed to protecting personal information. Personal information is handled in accordance with our Privacy Policy and the Privacy Act 1988 (Cth).
Complaints and Feedback
We welcome feedback and take all concerns seriously. If you have a complaint or would like to provide feedback about our services, you can contact us by using the contact form or by email.
If you are not satisfied with our response, you may contact the NDIS Quality and Safeguards Commission for independent support.
Changes to Services or Terms
JC Cornerstone Community Care Pty Ltd reserves the right to update these Terms & Conditions as required to reflect changes in legislation, policy, or service delivery. Updated terms will be made available on our website.
