Psychosocial Recovery Coaching

Psychosocial Recovery Coaching for confidence, recovery and everyday progress.

Recovery looks different for every person. For some, it may mean building a stronger routine. For others, it may mean reconnecting with community, strengthening confidence, managing daily challenges or feeling more in control of everyday life.

Psychosocial Recovery Coaching is an NDIS-funded support for people living with psychosocial disability. It provides practical, one-on-one support to help participants work towards recovery goals, build independence and feel more connected to the people, services and activities that matter to them.

At JC Cornerstone Community Care, our approach is compassionate, realistic and guided by each participant’s goals, choices and strengths. We walk alongside participants with steady encouragement, practical support and a strong belief that meaningful progress is possible.

Jacob Cook

FOUNDER & DIRECTOR
Founder's Story

A story built on lived experience.

JC Cornerstone Community Care was founded through lived experience and a deep belief in recovery, compassion and paying support forward.

Through my own challenges in navigating life’s ups and downs, I came to understand how important the right support can be. Over time, those challenges became a catalyst for growth. I realised that when I reached out, accepted guidance and stayed open to learning, I began to find my purpose.

That purpose is now reflected in JC Cornerstone Community Care. My experiences have shaped the way I support others: with honesty, realistic expectations, respect and a genuine belief that people can move forward.

I know the value of being heard, encouraged and supported by people who do not give up on you. That is the kind of support we aim to provide — practical, compassionate and grounded in real life.

Every person we support is treated with dignity, respect and care. We are here to listen, encourage and help participants take meaningful steps towards stability, independence and connection.

Together, we can.

What is Psychosocial Recovery Coaching?

Psychosocial Recovery Coaching is an NDIS support designed for people living with psychosocial disability. A Recovery Coach works alongside the participant to understand their needs, support their recovery goals, and help them build confidence, skills and connections in daily life.

Recovery Coaching can include practical support with planning, routines, motivation, coping strategies, service connection, community participation and communication with the participant’s support network.

A Psychosocial Recovery Coach can also help participants understand their NDIS plan, identify which supports may be useful, and connect with services that align with their goals.

At JC Cornerstone, Psychosocial Recovery Coaching is not a one-size-fits-all service. We take the time to understand the person, what recovery means to them and what kind of support will help them move forward in a realistic and meaningful way.

We understand that recovery is not always a straight path. Some days feel easier than others, and each person’s journey is shaped by their experiences, strengths, goals and support needs.

For one participant, recovery may involve building a routine that makes daily life feel more manageable. For another, it may involve reconnecting with family, participating in community activities, building confidence for appointments, or working with trusted providers.

At JC Cornerstone, we do not define recovery for you. We support you to explore what recovery means in your own life.

Our role is to provide compassionate support, realistic expectations and steady encouragement. We focus on what matters to you, helping you build the skills, confidence and connections that support a more meaningful life.

Psychosocial Disability and Mental Health Support

Recovery-focused support for psychosocial disability.

Psychosocial disability can affect daily life in many different ways. It may impact motivation, routines, relationships, communication, self-care, confidence, community participation and the ability to manage appointments or responsibilities.

JC Cornerstone supports participants whose day-to-day life may be affected by mental health-related support needs, including people living with conditions such as schizophrenia, schizoaffective disorder, bipolar disorder, severe depression, anxiety disorders, PTSD, OCD, agoraphobia, borderline personality disorder and other psychosocial or functional support needs.

Our focus is not on labels. It is on understanding the person, their goals, their strengths and the practical support they need to build stability, independence and connection.

Psychosocial Recovery Coaching can help participants:

Build confidence in daily life

Strengthen routines and structure

Connect with appropriate services

Set and work towards recovery goals

Improve community participation

Build coping strategies

Communicate with support networks

Understand and use their NDIS plan

Stay connected during periods of change

Participant & Family Feedback

Support that makes a difference through the hard moments.

“I care for my son who has a mental condition bordering on schizophrenia. The first years were very hard to cope with as I was on my own. Since becoming part of the NDIS we have been successfully supported by Jacob Cook, who has been nothing but an absolute asset to us.

He has helped in any way we have asked and has been there for my son during his troubling times. I couldn’t wish for a better support manager. He goes out of his way to help if we need things, and even if it’s not during his working time, he will still support us in any way he can. He truly goes beyond his job title.”

FAQs

Psychosocial Recovery Coaching FAQs

  • What does a Psychosocial Recovery Coach do?

A Psychosocial Recovery Coach works alongside a person living with psychosocial disability to support recovery goals, build routines, strengthen confidence, connect with services and improve daily life. The support is practical, person-centred and guided by the participant’s own goals.

  • Is Psychosocial Recovery Coaching funded by the NDIS?

Yes, Psychosocial Recovery Coaching is funded by the NDIS when it is included in a participant’s plan. If you are unsure whether your plan includes Recovery Coaching, we can help you check your funding and understand your options.

  • What is the difference between Support Coordination and Psychosocial Recovery Coaching?

Support Coordination focuses on helping participants understand and implement their NDIS plan, connect with providers and coordinate services. Psychosocial Recovery Coaching also supports NDIS navigation, but has a stronger focus on recovery, mental health-related support needs, routines, coping strategies and building confidence in daily life.

  • Do you support participants living with schizophrenia, bipolar disorder, PTSD or severe depression?

Yes. JC Cornerstone supports participants living with psychosocial disability and mental health-related support needs. This may include people whose daily life is affected by schizophrenia, schizoaffective disorder, bipolar disorder, severe depression, anxiety disorders, PTSD, OCD, agoraphobia, borderline personality disorder or other psychosocial support needs.

  • Can children and teenagers access Psychosocial Recovery Coaching under the NDIS?

Yes. While Psychosocial Recovery Coaching was originally designed with adults in mind, it is frequently funded for teenagers and younger participants. The NDIS allocates this support based on a participant's functional needs rather than a strict minimum age. For younger demographics (typically ages 12 to 25), a Recovery Coach focuses on age-appropriate capacity building. This includes developing daily routines, managing transitions to school or tertiary education, building social confidence, and working closely with the participant's family and wider support network.

Note: For children under 9, the NDIS generally prioritises early intervention therapies through the Early Childhood Approach.

  • Can family members or carers be involved?

Yes, where the participant wants this or where involvement is appropriate. We can work alongside families, carers, advocates, nominees and support teams while respecting the participant’s privacy, rights, choice and control.

  • Can Recovery Coaching help with community participation?

Yes. Recovery Coaching can support participants in building confidence in community access, reconnecting with meaningful activities, reducing isolation, and taking practical steps towards social and community participation.

  • Do you provide Psychosocial Recovery Coaching in Richmond, Bentleigh and Melbourne’s South East?

Yes. JC Cornerstone provides Psychosocial Recovery Coaching across Melbourne's inner city, north eastern and south eastern suburbs, and the Mornington Peninsula.

  • How do I get started with Psychosocial Recovery Coaching?

You can start by calling us, sending an enquiry or making a referral. We offer a no-cost initial Meet & Greet to understand your needs and explore whether JC Cornerstone is the right fit.
