Support Coordination

Support Coordination that helps you use your NDIS plan with confidence.

Understanding your NDIS plan is one thing. Knowing how to put it into action is another.

For many participants, choosing providers, coordinating services, understanding budgets and staying on top of appointments can feel confusing and overwhelming. This can be even more challenging when you are managing mental health concerns, psychosocial disability or changing support needs.

JC Cornerstone Community Care provides practical, person-centred Support Coordination to help you understand your plan, connect with the right providers and build confidence using your supports over time.

Our approach is grounded in clear communication, realistic guidance and a genuine commitment to helping people move forward with greater stability, independence and control.

Jacob Cook

FOUNDER & DIRECTOR
Founder's Story

A story built on lived experience.

JC Cornerstone Community Care was founded through lived experience and a deep belief in recovery, compassion and paying support forward.

Through my own challenges in navigating life’s ups and downs, I came to understand how important the right support can be. Over time, those challenges became a catalyst for growth. I realised that when I reached out, accepted guidance and stayed open to learning, I began to find my purpose.

That purpose is now reflected in JC Cornerstone Community Care. My experiences have shaped the way I support others: with honesty, realistic expectations, respect and a genuine belief that people can move forward.

I know the value of being heard, encouraged and supported by people who do not give up on you. That is the kind of support we aim to provide — practical, compassionate and grounded in real life.

Every person we support is treated with dignity, respect and care. We are here to listen, encourage and help participants take meaningful steps towards stability, independence and connection.

Together, we can.

What is Support Coordination?

Support Coordination is an NDIS-funded support that helps participants understand, implement and make the most of their NDIS plans.

A Support Coordinator works alongside you to help turn your plan into practical supports. This may include explaining your funding, connecting you with suitable providers, coordinating services, preparing for plan reviews and helping resolve issues when supports are not working as they should.

The goal of Support Coordination is not just to organise services for you. It is also about building your skills, confidence and independence so you can make informed decisions and have more choice and control over your supports.

At JC Cornerstone, Support Coordination is tailored to the person. We take the time to understand your needs, your goals and what support looks like in real life.

How We Can Help

How we can help with your NDIS plan.

Our Support Coordination service is designed to make the NDIS feel clearer, more manageable and more connected to your everyday life. We can assist with:

Understanding what is included in your NDIS plan

Explaining how your supports can be used

Connecting with suitable providers and community services

Coordinating supports across different providers

Helping you compare options and make informed choices

Identifying gaps in your current supports

Communicating with providers and support teams

Preparing for plan reviews or reassessments

Supporting you to work towards your NDIS goals

Building confidence to manage supports more independently

We can also work alongside families, carers, nominees, advocates and other professionals where appropriate, helping create a more connected and consistent support experience.

A Practical and Recovery-Focused Approach

Support that looks at the whole person.

At JC Cornerstone, Support Coordination is not just about services, rosters and appointments. It is about the person behind the plan.

We take a recovery-oriented approach, which means we look at your strengths, your goals, your lived experience and what meaningful progress looks like in your day-to-day life.

For some people, progress may mean building stronger routines. For others, it may mean reconnecting with community, finding the right support team, improving communication or feeling more confident when making decisions.

We understand that support needs can change over time. Mental health challenges, life transitions and changes in circumstances can all affect how someone uses their NDIS supports. Our role is to help reduce overwhelm, make the process clearer and support you to take practical steps forward.

Support Coordination for Psychosocial Disability

Support Coordination for participants living with psychosocial disability.

For participants living with psychosocial disability, navigating the NDIS can sometimes feel especially complex. Support needs may change over time, motivation may fluctuate, and it can be difficult to manage providers, appointments and everyday responsibilities all at once.

JC Cornerstone provides recovery-focused Support Coordination that helps participants understand what supports are available, connect with suitable providers and build a support network that responds to their goals and circumstances.

We recognise that recovery is not always a straight path. That is why our approach focuses on realistic progress, consistency, trust and support that meets people where they are.

Our Support Coordination may assist participants living with psychosocial disability to:

Understand and use their NDIS plan

Connect with mental health-informed providers

Coordinate practical daily living supports

Build routines and structure

Improve communication between support networks

Prepare for plan reviews

Stay connected with community supports

Work towards recovery and independence goals
