Support Work

NDIS Support Workers who help everyday life feel more manageable.

The right NDIS Support Worker can make a meaningful difference to daily life. Support Work is about more than completing tasks — it is about helping participants build confidence, stay connected and feel supported in the routines and activities that matter to them.

JC Cornerstone Community Care provides practical, person-centred NDIS Support Work across Melbourne's inner city, north-eastern and south-eastern suburbs, and the Mornington Peninsula.

Our support workers assist with daily living, personal care, community participation, appointments, skill development, health monitoring and building independence in a way that feels respectful, reliable and shaped around the participant’s goals.

Jacob Cook

FOUNDER & DIRECTOR
Founder's Story

A story built on lived experience.

JC Cornerstone Community Care was founded through lived experience and a deep belief in recovery, compassion and paying support forward.

Through my own challenges in navigating life’s ups and downs, I came to understand how important the right support can be. Over time, those challenges became a catalyst for growth. I realised that when I reached out, accepted guidance and stayed open to learning, I began to find my purpose.

That purpose is now reflected in JC Cornerstone Community Care. My experiences have shaped the way I support others: with honesty, realistic expectations, respect and a genuine belief that people can move forward.

I know the value of being heard, encouraged and supported by people who do not give up on you. That is the kind of support we aim to provide — practical, compassionate and grounded in real life.

Every person we support is treated with dignity, respect and care. We are here to listen, encourage and help participants take meaningful steps towards stability, independence and connection.

Together, we can.

What is NDIS Support Work?

NDIS Support Work provides practical assistance for participants who need help with everyday activities, routines, personal care, community access and building independence.

An NDIS Support Worker may support a participant at home, in the community, while attending appointments, during social or recreational activities, or while working towards daily living goals.

At JC Cornerstone, Support Work is delivered with respect, patience and a person-centred approach. We take the time to understand each participant’s preferences, goals, routines and support needs so that assistance feels practical, safe and comfortable.

The aim is not to take over. The aim is to provide the right level of support so participants can build confidence, maintain choice and control, and stay connected to everyday life.

Participant Feedback

Friendly support that builds connection and confidence.

“I have been seeing Jacob on a regular basis for the last few years. Jacob has been there for me in the good times, but also the not so good times.

I would highly recommend Jacob to the younger participants. He is a really cool cat, we have a lot of laughs and great conversations together. In addition, he has a background in fitness as a previous personal trainer. I have enjoyed going on a group run with him and long walks and talks also. Further, he has also helped me get involved in activities.

Please do yourself a favour and let Jacob help you!"
